3 Consent

1. I, the parent/guardian of the player named above, consent to their participation in the Balkan Hockey Championship 2026/27 season.
2. I understand that BHC is full-contact, body-checking hockey under the IIHF rules (§7.1) and I am aware of the associated risk of injury, including serious injury.
3. I confirm that the player is physically fit, has no medical contraindications and holds valid sports insurance covering ice hockey (§3.7). I understand that the League does not insure players.
4. I agree that the player wears the full set of mandatory IIHF-certified equipment, including a visor or cage, which is mandatory until age 18 (§3.8).
5. I consent to first aid and, if necessary, emergency medical care being provided to the player during League game rounds.
6. I have read the BHC 2026/27 Regulation and the Fair Play Code (§12) and accept them on the player's behalf.
7. I consent to matches being filmed and broadcast, and to the League using photos and video featuring the player in its media without separate payment.
8. I consent to the League publishing the player's name, position, jersey number, match statistics (games, goals, assists, penalty minutes, goalie statistics) and match video and photos, and sharing them with the League's official partners (e.g. statistics and recruiting platforms) for display in the player's profile. The player's contact details and mine are not shared without separate consent.
